Teaching Assistant Registration and Payment Forms

It is COMPULSORY to complete the all the forms mentioned below. If you fail to complete any of these forms, your contract will not be arranged and payment can not be processed.

Registration Forms
Step 1
You will need to complete this form if you wish to be considered for Teaching Assistant.
- Details from this form will be sent to Laboratory Conveners, who will make a selection on your case depending on your skills and experience.
- You will be notified via an email to the email account you indicated in the above form or any other means as appropriate.

- You will need to complete this form only ONCE an academic year
Step 2
ONLY for students who have been selected and require a Teaching Assistant contract.
To complete this form, you need to following information:
- Name of Laboratory Convenor(s) you will be working for
- How many hours per semester you will work for each Laboratory Convenor
- Total hours you will work for each semester
- You will need to complete this form EVERY SEMESTER you will take TA responsibilities
PR14 (Monthly Timesheet)
Step 4
It is COMPULSORY to complete the PR14 form EVERY MONTH, in order to get paid.
- If you will submit it once a semester, you may be taxed and will have to wait until end of tax year to claim the tax back.
- This is not a good practice for the department's record keeping and we highly ENCOURAGE to submit form each month, even if you have worked only for few hours in a month.
- The completed form must be submitted by the 4th of each month at the designated place.
- Please ensure that you get your form signed by respective lab convener.
- Chemistry students should submit their forms in the bottom drawer of the wooden box outside of room G.020 (Chemsitry Education Office Reception).
- DTC/ CDT students need to submit their forms at the DTC/ CDT office in chemistry building.
- Please complete all information requested on the form. If your form is not completed, it will not be processed.
- No notification could be produced in this instance, however, you will be able to complete a new timesheet and be able to submit it next month.
- If you need help in completing the form, please contact Laboratory Convenor or TA Coordinator.
- You should submit only one PR14 per month. Please note that if you submit more than one form, it must be stapled.
