Before you conduct any experiment in the laboratory, you MUST complete the "Chemical Risk Assessment Form" and have it signed/authorised by your supervisor (and if necessary, the Department's Safety Advisor).

Authorisation Process - Note for Students / Researchers / Supervisors

  1. Please download and complete the form electronically.
  2. Once completed, please email it to your supervisor (or his/her designate in their absence).
    • To minimize delays, the subject of the email must be in the following format: CRA Approval: Short Experiment Name or Description.
  3. Your supervisor will inspect the form to make sure it is completed correctly. You are authorised to proceed with the experiment once you received the form back from your supervisor via email (also copied to CRA.Chemistry@manchester.ac.uk).
    • To minimize delays, the subject of the email must be in the following format: CRA Approval: Short Experiment Name or Description
  4. For certain types of experiment, your supervisor needs to forward the form to the School Safety Advisor. You are authorised to proceed with the experiment once you received the form back from the Advisor.

  5. Once the form is emailed back to you from your supervisor (or the Safety Advisor), you must retain a copy and display a printed copy close to where the experiment is to be conducted.
